Nasopharyngeal carcinomas: prognostic factors and treatment features.

چکیده
PURPOSE We retrospectively evaluated the clinical, radiological and pathological features determining the prognosis of patients with nasopharyngeal carcinoma in Ankara Oncology Hospital, Turkey. MATERIAL AND METHODS Two hundred and fifty-nine patients, 74 women and 185 males with nasopharyngeal carcinoma were treated between 1993 and 2008. All imaging data including CT and MRI were reevaluated according to the criteria which determine parapharyngeal, oropharyngeal, nasal, skull-base (bone)/sinus, infratemporal fossa, orbit, intracranial involvements and lymph node metastasis by our radiologists. The patients were restaged using the AJCC 2002 classification with these new radiological findings and clinical data base. We evaluated prognostic factors using univariate Kaplan- Meier and multivariate Cox regression analyses. Gender, age (40-year cut-off), histology, T- and N-stage, tumor size, regional involvement, radiotherapy and/or chemotherapy and response to therapy were studied as variables. RESULTS Five-year disease-free and overall survival rates were 45 +/- 4 % and 72 +/- 3 % , respectively. We found that age, gender, WHO type, radiotherapy and/or chemotherapy, N-stage and response to therapy were significant prognostic factors on disease-free survival and overall survival. In the chemo-radiotherapy group, we did not detect any survival difference between patients given four or fewer chemotherapy courses. CONCLUSIONS Radiotherapy improved survival but chemotherapy, in the neoadjuvant and adjuvant setting, had no added effect to radiotherapy. N-stage and response to treatment were the most important independent predictors on survival. Age, gender, type, therapy and bone/sinus involvement were among the predictive factors on multivariate analysis, as well. Histopathological Evaluation and Analysis of Immunohistochemical Expression of Bcl-2 Oncoprotein in Colorectal Carcinoma
Abstract Introduction: Colorectal cancer is the third most prevalent malignancy with high mortality rate, necessitating markers that predict survival and guide the treatment. Previous studies have examined the immunohistochemical expression of Bcl-2, an apoptotic marker, in colorectal carcinoma, but results have been contradictory. Objectives: To evaluate the histopathological featu...
متن کاملCorrelations between expression of epidermal growth factor receptor (EGFR), phosphorylated EGFR, cyclooxygenase-2 and clinicopathological variables and treatment outcomes in nasopharyngeal carcinomas.
BACKGROUND To evaluate immunoexpression of epidermal growth factor receptor (EGFR), phosphorylated EGFR (pEGFRTyr1068), and cyclooxygenase-2 (COX-2) and analyze their prognostic utility in nasopharyngeal carcinomas (NPC). METHODS We used a retrospective review of charts and tissue specimens. The immunoexpression levels of EGFR, pEGFRTyr1068, and COX-2 were semiquantitatively assessed by the H...
متن کاملExpression of Epidermal Growth Factor Receptor and the association with Demographic and Prognostic Factors in Patients with Non-small Cell Lung Cancer
Introduction: Growth, proliferation, survival, and differentiation are the prominent characteristics of cells, which are affected by cancer. Epidermal growth factor receptor (EGFR) plays a pivotal role in the effective control of these features. Given the significance of EGFR signaling pathway in non-small cell lung cancer (NSCLC), EGFR expression is influential on these cell characteristics. I...
متن کاملClinicopathologic idiosyncrasies of nasopharyngeal cancer in a moderate-risk Mediterranean region
Cancer of the nasopharynx displays an unparalleled skewness of its epidemiologic, pathogenic and clinico-prognostic characteristics depending on the geographic location. Between the endemic and sporadic forms, which occur in Southeastern Asia and Northern America, respectively, intermediate incidence is noted around the Mediterranean. This study describes the patterns of the disease affecting t...
متن کاملThe Relationship Between Fibroblastic Growth Factor Receptor-1 (FGFR1) Gene Amplification in Triple Negative Breast Carcinomas and Clinicopathological Prognostic Factors
Background & Objective: In Triple-Negative Breast Cancers (TNBCs), estrogen receptor (ER), progesterone receptor (PR) and HER2/neu genes are not expressed. Fibroblastic Growth Factor Receptor-1 (FGFR1) gene product is a protein that acts as a receptor of thyrosin kinase. It plays a role in the proliferation, differentiation, and migration of malignant cells. The objective was t...
